Form Instructions: Carefully read each statement. If the statement is true for you check the box next to the corresponding statement, if the statement is false for you do not check the box next to the statement. After completing the form click the button marked "Am I Qualified?" at the bottom of this page, to determine whether or not you are eligible to continue the recruitment process.
 

1  I am at least 21 years old.
2  I am a US born citizen or have been naturalized.
3  I have a high school diploma or GED.
4  I have no felony convictions.
5  I have no convictions, misdemeanor or city ordinance, involving family violence.
6  I have no history involving the sale and/or distribution of an illegal substance as an adult as defined by Georgia law or of a significant nature as a juvenile.
7  I have a valid driver's license (license may be from any U.S. state).
8  I have not used marijuana within 2 years of the date of application.
9  I have not used any Schedule I narcotics within 10 years of the date of application. Prior use will be determined on a case by case basis.
10  I have not used any controlled substance (Schedule II or III narcotics) within the last 5 years of the date of application.
11  I have not used any illegal steroids within 3 years of the date of application.
12  I have no misdemeanor convictions within 3 years of the date of application, to include DUI.
13  I have not made any false statements or knowingly provided false information on the application or any document related to the application process.
